LOCATION: LifeTime Fitness-1616 West Ruby Dr., Tempe, AZ 85285 Tel 480-705-8855.

Olympic Format: June 4-6, 2010. Start at 3pm on Friday, June 4th.

ENTRY FEES: $30 for Olympic event.

ENTRY DEADLINE: Early entry deadline (postmarked on or before) May 22, 2010. Late Entry deadline must be received in the State Games Office by May 29, 2010. Any entry after May 29, 2010 will be accepted only with Commissioners approval and will be based on space availability- add $10 late fee.

DIVISIONS*:
(players may enter 1 event ) USSRA rules will apply. Protective eyewear mandatory.
Mixed 5.5, 5.0, 4.5,4.0, 3.5,3.0 and 2.5.
• Olympic draw guarantees 3 matches with a maximum of 5
